All financial arrangements are done at our dental office and there will be no hidden costs and no additions after your treatment plan is confirmed and treatment begins. Please understand that your insurance is a contract between you, your employer and the insurance company. As dental care providers, our relationship is with you, and not your insurance company. All charges are your responsibility. From the date the services are rendered, you are ultimately responsible for the balance of your account, regardless of your insurance status.

Insurance Information:

Our fees are accepted by most insurance companies, and therefore are covered up to the maximum allowance determined by each carrier. However, not all services are covered by insurance benefits, in every contract. Some insurance companies select certain services they will not cover. There is always a deductible.

Plan Information: If your plan is a PPO type plan with "out-of-network" benefits, our office accepts benefits that your insurance company provides. Depending on your particular plan you may have full coverage for some procedures and partial coverage for others.

We are supporters of our troops and their families, and gladly accept United Concordia Dental.

Claims:  Filing of insurance claims is a courtesy that we extend to our patients.
With current and accurate insurance information, we are also able to generate a customized treatment plan estimate that summarizes your estimated insurance payment and your out-of-pocket costs for treatment that is required and has been agreed upon.
